Our completely electronic medical record allows us to provide you with the highest quality care and allows for optimal communication with your other health care providers. In addition, we know that every patient is different and has specific needs, and we will strive to provide you with personal, individualized attention.
Please prepare for this by bringing:
Health Insurance Card/Information
A friend or family member to take notes/help with questions
A list of all medications you are taking
A list of your questions /concerns
A copy of your XRAYS / CT scans (not just the reports - get the actual films as well)
Copies of records from doctors related to your sinuses/nose/allergies/lungs.

We strive to provide same day care for our patients when necessary. Please notify us at least 24 hours in advance of any cancellation as this will allow us to give your appointment slot to another patient who may need to be seen expeditiously.
